100 Comparative immune landscapes of lichen planus and ICI-associated lichen planus by single cell spatial proteomics

Abstract
Lichen planus (LP) is a chronic autoimmune mucocutaneous disease characterized by pruritic skin lesions and painful mucosal ulcers, affecting nearly 1 million people in the United States. There are currently no FDA-approved therapies, and existing treatments are often inadequate for severe disease. ICI-associated lichen planus (ICI-LP) represents a related but distinct entity that arises in patients receiving immune checkpoint inhibitors for cancer, with overlapping clinical features but potentially divergent immunopathogenic mechanisms. To better define the immune infiltration driving these conditions, we performed single-cell spatial proteomics on biopsy specimens from patients with LP and ICI-LP, enabling a direct comparison of their immune architecture. Using a 30-plex panel of metal labeled antibodies, we performed multiplex ion beam imaging time-of-flight (MIBI-TOF) on archived biopsies from patients with lichen planus (n=5) and ICI-LP (n=5). A total of 32 unique cell populations were enumerated from the 206,318 cells, revealing a unique structured immune cell composition. Our analyses revealed that ICI-LP lesions harbored a greater abundance of naïve cytotoxic CD8 T cells, as well as increased infiltration of both M1 and M2 macrophages, compared to LP. These findings highlight shared and unique immune signatures between LP and ICI-LP and suggest distinct immunopathogenic drivers that may inform therapeutic strategies. Next steps include integration of single-cell spatial transcriptomics currently underway to identify unique inflammatory pathways that distinguish LP from ICI-LP. Understanding these mechanisms is critical for the development of targeted, effective treatments for patients with both classic LP and immunotherapy-induced disease.
