(1123) Association Between Donor-Derived Cell-Free DNA Levels and Cardiac Pressures in Pediatric Heart Transplant Recipients

Abstract
The goal of this study was to assess the relationship between donor-derived cell-free DNA (dd-cfDNA) and cardiac pressures following pediatric heart transplantation (HTx).
This exploratory retrospective study included 24 pediatric HTx recipients with dd-cfDNA (AlloSure®, CareDx) levels collected between June 2021 and August 2022. Patients with concurrent acute cellular rejection (grade 2R or higher) or antibody mediated rejection (pAMR1 of higher) were excluded from this analysis. Cardiac pressures were assessed during right heart catheterization. The association between dd-cfDNA and cardiac pressures was assessed using Pearson's correlation.
The study cohort was primarily white (75%) and male (50%) with a median (IQR) age at time of dd-cfDNA draw of 17 (7-19) years. A total of 45 dd-cfDNA samples were paired with right heart catheterization and included in this analysis, with dd-cfDNA levels drawn a median (IQR) of 753 (374-1241) days post-transplant. The dd-cfDNA levels were positively correlated with right sided cardiac filling pressures (Table). Pulmonary capillary wedge pressure did not show a positive correlation.
Elevated dd-cfDNA levels were positively correlated with elevated right sided cardiac filling pressures in this cohort of pediatric HTx recipients not experiencing allograft rejection. Wedge pressure did not demonstrate a statistically significant correlation coefficient. Additional studies with larger sample sizes are needed to confirm these findings.
