277 The impact of strabismus and strabismus surgery on quality of life of adult patients

Abstract
Introduction
Strabismus affects roughly 4% of US adults. For many, strabismus causes diplopia and decreased depth perception, leading to visual impairment. Poor eye contact has led to this population being perceived as less intelligent, attractive, and trustworthy, and less likely to be hired, promoted, or married. Strabismus surgery has been considered cosmetic by some, however, surgery can improve quality of life and vision.
Methods
This prospective study enrolled adult patients with strabismus. The National Eye Institute’s Visual Function Questionnaire-25 was administered and scored into 12 subscales assessing quality of life and vision. Patients who underwent surgery were also evaluated post-operatively. Pre- and postoperative measurements were recorded to determine surgical optimality.
Results
118 patients were enrolled. All showed decreased quality of life and vision scores compared to the reference group (P < 0.001). The most impacted score was mental health (P < 0.001). There was no significant trend in questionnaire scores in relation to sex, age, strabismus type or size, or diplopia. Of the 67 patients who underwent strabismus surgery, 39 have completed the postoperative questionnaire. Patients surveyed post-operatively had significant improvement in quality of life and vision scores, even those with suboptimal surgical outcomes (P < 0.001). Postoperative score improvement did not significantly differ between patients with optimal and suboptimal outcomes. (P > 0.05).
Conclusion/Relevance
Strabismus negatively impacts patients’ vision and quality of life. Strabismus surgery, regardless of surgical outcome, significantly increased patients’ vision and quality of life. Strabismus surgery should not be considered cosmetic, as it restores normal appearance, binocularity, and improves patient quality of life.
