52109 Characterization of Melanoma Referrals to UIHC and Impact on Patient Outcomes

Abstract
The 1.2 million Iowa residents who live in rural areas present unique challenges in the diagnosis and treatment of melanoma across the state. Studying the demographic and geographical challenges in melanoma diagnosis, referral, and treatment in the state of Iowa will provide insight into the management of melanoma in the rural United States. We aim to analyze provider practice trends by characterizing melanoma referrals from rural and urban communities to the University of Iowa Hospitals and Clinic (UIHC), a tertiary referral facility. We will explore the association with clinical outcomes to identify areas of concern while optimizing melanoma referral guidelines. A total of 234 patients with diagnoses of melanoma from 2016-2021 were identified through TriNetx and included in this study. Most patients in this data set received their melanoma diagnosis outside of UIHC (65.2%), however, 85% of all patients received treatment for their melanoma at UIHC. Dermatologists made up 74.2% of all diagnosing providers.The data demonstrated an upstaging from initial clinical to final pathological staging after excision was performed, possibly due to inaccurate initial Breslow depth due at the time of diagnosis. Stage II diagnosis decreased by 4.2%, while Stage III and Stage IV increased by 3.2 and 1.0 %, respectively, after procedural intervention.Non-UIHC facilities performed more SLNBs at any Breslow depth (51% vs. 31%) and more when Breslow depth < 1mm (23.08% vs. 3.85%). Rural melanoma care in the state of Iowa may correlate with higher rates of SLNB, especially under 1mm.
