A Comparative Analysis Of Outcomes Of Implantable Cardioverter Defibrillator In Patients With Heart Failure And Left Bundle Branch Block And Right Bundle Branch Morphology

Abstract
Most data on outcomes of implantable cardioverter defibrillator (ICD) in heart failure patients come from large clinical trials analyzing the devices’ long term outcomes. However, there is a paucity of real world data on short term outcomes, especially in comparing the effects of Implantable Cardioverter Defibrillator (ICD) in patients with HF and left bundle branch block (LBBB) and right bundle branch block (RBBB) morphology respectively.
Using the 2016-2017 Nationwide Inpatient Sample, we performed a cross-sectional analysis and identified a weighted estimate of 73,989 hospitalizations with an ICD-10 CM procedure code of ICD and subsequently classified them using the Diagnosis Codes into those having LBBB and RBBB morphology. Primary outcome was all cause in-hospital mortality and secondary outcomes were length of stay (LOS), and atrial fibrillation (AF). A logistic regression model adjusted for age, sex, race, admission type, hospital characteristics and relevant comorbidities was performed to match for possible confounders.
Out of a total of 73989 admissions with ICD, 5525 had LBBB and 3125 had RBBB. No differences were found in all cause in-hospital mortality (aOR = 1.07, 95% CI 0.45 - 2.52; p = 0.87), the mean LOS (aOR = 1.01, 95% CI 0.99 - 1.02; p = 0.16) and AF (aOR = 1.05, 95% CI = 0.94 - 1.16; p = 0.33).
While evidence cites greater mortality benefits from CRT-D implantation in patients with LBBB compared to RBBB, no significant differences were found in outcomes of ICD therapy between these two groups.
