Abstract
IntroductionVentricular fibrillation is one of the most common presentations in the acute phase of ST-elevation myocardial infarction (STEMI) and it markedly increases the in-hospital mortality. However, it is suggested that ventricular tachycardia (VT) and ventricular fibrillation (VF) occurring in the first days of STEMI is a poor predictor of mortality even after hospital discharge while other studies said that patients who survive to hospital discharge are believed to have a similar long-term prognosis to that of patients who do not experience ventricular arrhythmias during the acute phase of STEMI.PurposeTo determine the prognostic impact of VF and hemodynamically unstable VT on the in-hospital major adverse cardiovascular and cerebrovascular events (MACCE) as well as the one-year all-cause mortality.MethodsThe data of a total number of 1415 of patients presented with STEMI in a tertiary care center equipped with a 24-hour ready catheterization laboratory in the period from June 2014 till June 2017 was reviewed and follow-up as regards 1-year all-cause mortality.ResultsA total number of 1415 of patients were divided into 2 groups, a no VT group had 1294 patients and VF/VT group had 121 patients. History of old myocardial infarction (MI) was 8.35%, 27.27% in VT/VF group which was statistically significant (P<0.001), GRACE score was 90±25 and 144±21 respectively which was statistically significant (P<0.001). Comparing in-hospital total MACCE between both groups, it was statistically significant (P<0.001) while 1-year all-cause mortality was statistically significant (P=0.06).ConclusionVT/VF during the acute phase of STEMI were associated with higher in-hospital MACCE while no difference was present in 1-year all-cause mortality.
