Abstract

IntroductionHeart Failure with Preserved Ejection Fraction (HFpEF) is a heterogeneous condition. Defining phenotypic subgroups of patients with HFpEF and the impact of comorbidities in HFpEF is important. Chronic Obstructive Pulmonary Disease (COPD) often coexists with HFpEF, but the impact of this co-morbid condition is poorly understood.MethodsWe studied 159 subjects with HFpEF, COPD or both and compared those with COPD alone (n=48), HFpEF alone (n=79) and HFpEF + COPD (n=32). We used MRI to assess cardiac structure and function and thoracic aortic pulse wave velocity (stiffness). Aortic characteristic impedance was measured with a combination of arterial tonometry and phase-contrast MRIResultsHFpEF patients with or without COPD exhibited a greater prevalence of female sex, obesity, furosemide, nitrate, beta-blocker and aspirin use. There were no differences in age, sex, or BMI between HFpEF patients with vs. without COPD, but patients with COPD + HFpEF were more often African-American. Compared to the other groups, subjects with HFpEF and COPD demonstrated a greater ratio of mitral inflow to annular early diastolic velocities (E/e’), more concentric geometry, a greater LV mass, and LV extracellular volume index (P=0.002). Patients with comorbid HFpEF + COPD also exhibited a much greater thoracic aortic stiffness assessed by PWV (P=0.0044), and a greater aortic characteristic impedance (ANOVA P=0.0069).ConclusionsIndividuals with co-morbid HFpEF and COPD exhibit a greater degree of systemic large artery stiffening, LV remodeling and LV fibrosis than those with either condition alone.
