Cardiac Troponin T Detectable with a Highly Sensitive Assay is Associated with Atrial Fibrillation in the General Population

Abstract
Background: Biomarkers predicting atrial fibrillation (AF) have not been established. New high-sensitive cardiac troponin T (hsTnT) assay enables us to detect myocardial damage more sensitively. Purpose: We examined whether hsTnT levels were associated with AF in the general population. Methods and Results: We analyzed 224 subjects (67.4±9.4 years, 94 males) who received a health examination in a fishing community (Uku town, Nagasaki) in 2010. Twelve patients (5.4%) were diagnosed as having AF based on electrocardiogram, and 132 (58.9%) were positive for hsTnT assay. AF group had greater hsTnT, BUN, and echocardiographic left atrial dimension (LAD) and lower estimated glomerular filtration rate (eGFR) than non-AF group. Logistic regression swed that LAD, hsTnT, and eGFR (inversely) were associated with AF. Step-wise logistic regression demonstrated that LAD (relative risk [RR]=1.314 (1.177-1.605, [95%CI]), p<0.001) and hsTnT (RR=4.576 (1.339-15.644), p=0.015) were independent risk factors for AF. The prevalence of AF closely and positively correlated with hsTnT levels (p=0.003 for trend). After adjustment for significant backgrounds (age, systolic blood pressure, eGFR, uric acid, LAD, and left ventricular mass index), estimated average of hsTnT was significantly greater in AF group than in non-AF group (p=0.018). Conclusion: In this population, hsTnT was an independent risk factor of AF. Moreover, hsTnT levels were positively associated with AF. hsTnT may be a candidate for the biomarker predicting AF.
