Conducting a Needs Assessment of Surgery Residents Using SCORE Questions

Abstract
INTRODUCTION: The Surgical Council on Resident Education (SCORE) created an online resource covering topics general surgery residents are expected to know for board certification. Colorectal surgery is a key component of general surgery education. Our aim was to determine colorectal knowledge gaps at both beginning and end of the colorectal rotation to improve resident education.
METHODS: Two quizzes were created from SCORE questions, divided between large intestine and anoretal topics. Quizzes were administered the first and fourth weeks of the colorectal rotation for one academic year. Following quiz administration, a colorectal surgeon reviewed answers with residents. Order of quiz administration was reversed halfway through the study period.
RESULTS: Thirty-two residents were given pre- and post-rotation quizzes. There was a statistically significant increase in scores from pre- to post-rotation (65%±14 vs 74%±9, p=0.0004). On pre-rotation quizzes, junior (PGY1-2) residents scored lower (59%±14) than senior (PGY3-5) residents (70%±13, p=0.034). Both groups had statistically significant increases in post-rotation scores (juniors 71%±11, p=0.012; seniors 77%±6, p=0.016). There was no difference between pre- and post-rotation scores for large intestine questions (73%±13 vs 76%±11, p=0.18); however, there was significant improvement in anorectal questions (pre-rotation 58%±18 vs post-rotation 75%±15, p<0.0001). There was no difference between the pre- and post-rotation scores after quiz order was reversed (p=0.25).
CONCLUSION: There was significant improvement in overall scores and anorectal questions, however there remains room for improvement. Both junior and senior residents would benefit from focused colorectal education. We plan to implement a curriculum for residents based on these results.
