Constipation in pregnancy - Fact or myth?

Abstract
OBJECTIVE: To prospectively assess constipation prevalence and risk factors in pregnancy.
METHODS: We performed a longitudinal study by enrolling healthy pregnant women during the first trimester. At each trimester and at 3 months postpartum, participants completed a self-administered bowel symptom questionnaire as well as physical activity and dietary fiber intake measures and a prospective 7-day stool diary. Constipation was defined using the Rome II criteria. Logistic regression was used to identify factors associated with constipation in later pregnancy.
RESULTS: One hundred four women were enrolled with mean (SD) age of 28 (5) years; 54% were nulliparous and 92% Caucasian. Constipation prevalence and diary stool frequency results (median [interquartile range]) are presented in Table 1. Overall, 51% (95% confidence interval [CI] 39 – 62%) of women were constipated during pregnancy. Factors associated with constipation in later pregnancy included first-trimester constipation (odds ratio [OR] 3.9, 95% CI 1.18 –12.85; P.02) and, although not statistically significant, dietary fiber intake (grams per day) (OR 0.91, 95% CI 0.80 –1.03; P.12).
CONCLUSION: Constipation, based on standardized Rome II criteria, is common throughout pregnancy. Stool frequency alone is insufficient for diagnosing constipation.
