Evaluating Age of Diagnosis and Cervical Cancer Screening Overtime

Abstract
OBJECTIVES: In recent years, hysterectomy rates have been significantly decreasing due to novel, non-surgical ways managing benign gynecologic indications for hysterectomy. As a result, as modern women age, they are more likely to have a uterus and cervix in situ. Cervical cancer guidelines currently discontinue screening at the age of 65, if appropriate negative screening has been documented, based on historical data. Our aim was to determine if the age of diagnosis in cervical cancer is increasing in order to inform whether consideration should be given to extending cervical cancer screening beyond age 65.
METHODS: A retrospective cohort of women diagnosed with cervical cancer at one institution between January 1986 and September 2017 were analyzed. Data included demographic variables, last cervical cancer screening, age of diagnosis and cancer information.
RESULTS: Our cohort had a total of 853 women with cervical cancer, 92 of whom were over the age of 65. There was a significant increase in age at diagnosis over time by 0.20 years per year. The age of diagnosis increased from 42.8 years of age in 1986, to 48.7 years of age in 2016 (p <0.01). Over time, there was not a significant difference in percentage of women diagnosed over the age of 65 years, with 12.5% in 1986 versus 14.6% in 2016 (p=0.18). The mean time of a patient’s last pap smear before diagnosis was seven years (+/- 8 years).
CONCLUSIONS: At our institution, the age of diagnosis for cervical cancer is increasing over time. However, at this time, there was still no significant difference in the percentage of women diagnosed over the age of 65. It may require longer follow-up of outcomes of current screening guidelines to identify if the percentage of older women diagnosed with cervical cancer increases.
