Abstract
Non-pharmacological treatments are considered a first-line approach to the management of chronic pain. Exercise is recommended in clinical practice guidelines for nearly all chronic pain conditions. Despite this, individuals with chronic pain often report increased pain with exercise. Clinical application using an optimal individualized approach to exercise prescription for chronic pain conditions is proposed to provide improved outcomes and adherence. Understanding how exercise produces analgesia as well as barriers and facilitators to exercise in individuals with chronic pain will assist in developing an individualized exercise program as well as adapting the program to individuals who are resistant to treatment or experience worsened pain with exercise. Adherence is a key factor in effectiveness of an exercise program with greater adherence associated with greater improvement in pain, psychological factors, strength and disability. Dr. Sluka will review 1) the mechanisms and effectiveness of exercise for pain relief, 2) provide approaches and strategies for individualization and adaptation of the exercise program, and 3) review strategies to address barriers and enhance facilitators to exercise prescription in individuals with chronic pain.
