Higher Blood Pressure Variability is Associated with Impaired Vascular Endothelial Function in Healthy Middle‐Aged/Older Normotensive Adults

Abstract
High blood pressure variability (BPV) is associated with increased cardiovascular disease risk in middle‐aged and older adults (MA/O). In rodents, BPV is inversely related to nitric oxide‐mediated endothelium‐dependent dilation (EDD), but it is unknown if BPV is associated with EDD in MA/O. High [HFSBP] and low [LFSBP] frequency systolic (mmHg2) and time domain (SD of SBP, SBPvar; and diastolic BP, DBPvar) intra‐brachial artery BPV and brachial artery flow‐mediated dilation (FMD) were determined in 18 healthy MA/O. FMD was 57% lower (3.5±0.4 vs. 8.1±0.8%Δ, P<0.01) in MA/O with higher BPV (n=9; 6F/3M; age=62±1y; HFSBP=13.0±2.4; LFSBP=3.6±0.6; HFSBP/LFSBP=0.29±0.03; SBPvar=5.4±0.4 SD; DBPvar=3.4±0.2 SD) than MA/O with lower BPV (9; 6F/3M; 60±1; 2.4±0.4; 2.6±0.6; 1.04±0.16; 3.6±0.2; 2.1±0.2), whereas endothelium‐independent dilation (brachial artery dilation to sublingual nitroglycerin) was similar (26.0±1.7 vs. 28.2±2.6%Δ, P=0.55). The groups did not differ in resting BP (123±4/73±2 vs. 114±5/68±2 mmHg), body mass index, fasting plasma glucose or lipids, maximal oxygen consumption or baseline brachial artery diameter (3.33±0.18 vs. 3.29±0.15, P=0.87). In the overall group, FMD was related to HFSBP (r=−0.48, P<0.05) and LFSBP/HFSBP (r=0.72, P<0.05). Our results indicate that healthy normotensive MA/O with higher BPV have impaired EDD compared with their peers with lower BPV. Fellow of CNPq‐Brazil (Melo, RC).
