Abstract
IMPACT OF MEAN PLATELET VOLUME ON ST RESOLUTION FOLLOWING THROMBOLYSIS IN ACUTE ST ELEVATION MYOCARDIAL INFARCTION: A SYSTEMATIC REVIEW AND META-ANALYSIS

Abstract
Background
Changing in size, density, and reactivity of platelets play an important role in occurrence and progression of myocardial infarction. Increased mean platelet volume (MPV) increases the incidence of fibrinolytic failure in ST segment elevation myocardial infarction (STEMI). Electrocardiographic resolution of a ST elevation can be used as measure of reversal in the ischemic injury to the myocardium. We aimed to assess the association of MPV and the resolution of ST segment (STR) following thrombolysis.
Methods
We comprehensively searched the databases of MEDLINE and EMBASE from inception to October 2018. Studies that reported the admission MPV and the STR were included. If the STR was resolved ≥ 50%, the patients were classified as the “Responders” and if the STR was below 50%, they were classified as “non-Responders”. Data from each study were used to calculate mean difference (MD) and 95% confidence intervals.
Results
Three studies from 2013 to 2015 were included in this meta-analysis involving 703 subjects with STEMI who got treated with thrombolysis (379 Responder and 324 Non-Responder). Higher MPV was associated with impaired STR following the thrombolysis. (pooled MD= 1.18fL, 95 % confidence interval: 1.02-1.33fL, p<0.001, I2=13.5%).
Conclusion
Our study suggest that higher MPV at admission was associated with lower STR. MPV at admission might be useful in the prediction of impaired STR and in planning the need for adjunctive therapy to improve outcome of STEMI after thrombolysis.
