IP09-02 PROPHYLACTIC ANTIBIOTIC IN PUBERTAL TESTICULAR TORSION: IS IT JUSTIFIED?

Abstract
INTRODUCTION AND OBJECTIVE:
To determine if prophylactic antibiotics for urgent pubertal testicular torsion (TT) surgery is necessary and describe the number needed to treat (NNT) to prevent a surgical site infection (SSI). We sought to describe any factors that may increase the risk of SSI.
METHODS:
Retrospective chart review was conducted including 930 pediatric patients with clinically diagnosed acute scrotum between 2014-2023. Exclusion criteria included prepubertal TT and no follow-up after surgery. Univariate and multivariate analyses were performed in patients with cases (SSI) and controls (No SSI), to identify potential risk factors and determine the utility of antibiotic prophylaxis.
RESULTS:
Of the 930 patients identified, 447 met the inclusion criteria. Of those, 341 (76.2%) received prophylactic antibiotics. There were no significant differences in age, hours of pain at presentation, orchiectomy rates or urine culture results between groups. There was a statistically significant difference in the use of antibiotic prophylaxis by provider (p=0.001). The incidence of an SSI was 2.35% in the prophylaxis group vs 2.83% in the non-prophylaxis group (p=0.78). The NNT to prevent one SSI with antibiotic prophylaxis was 206 patients. In univariate and multivariate analyses, no significant clinical factors were identified that would increase the risk of SSI.
CONCLUSIONS:
Based on our analysis, we found no benefit from antibiotic prophylaxis in preventing SSIs in pubertal acute scrotum. In our cohort, no risk factors were identified that would change the rationale for antibiotic prophylaxis use for pubertal TT surgery.
