Influence of health insurance coverage on outcomes in pancreatic and liver surgery

Abstract
Introduction
US healthcare and insurance reform are topics of continued debate. While multiple studies have agreed that insurance coverage is an important determinant of access to care and cancer specific outcomes, data is lacking regarding effect of insurance on pancreatic and liver surgery outcomes. This study looks to identify if there were differences in surgical outcomes based on insurance coverage and other socioeconomic issues.
Methods
The Nationwide Inpatient Sample (NIS) was used to identify patients who underwent a pancreatic or liver surgery between 2003-2009. Data were collected on demographics, socioeconomic status and racial risk factors. Logistic regression models were used to evaluate complication rates related to these procedures.
Results
Among 29,010 patients included (37.4% males, 74.6% white), 11,541 (41.2%) patients who received surgical treatment were covered under a Medicare, 13,612 (48.2%) were covered under private insurance. Medicare patients were more likely to have postoperative complications (OR, 1.273, 95% CI 1.087-1.492, p=0.0028). Furthermore in hospital mortality rate was higher in Medicare patients compared to patients with private insurance (OR 1.753, 95% CI 1.320-2.329, p=0.0001). Medicare patients had a longer length of stay (OR 1,292, 95% CI 1.153-1.448, p<0.0001). Additionally, cost of health services was higher for patients with Medicare ($33,504.00±800.38) or Medicaid ($37,118.00±1933.84) coverage compared to those with private insurance ($27,373.00±686.50) (p=0.0005).
Conclusions
Health insurance coverage is associated with perioperative outcomes for patients undergoing pancreas and hepatic surgery. Addressing the causes for these findings is essential to ensure that all patients undergoing pancreatic or liver surgery receive access to high quality care.
