LEARNING CURVE FOR ENDOSCOPIC SLEEVE GASTROPLASTY: A SYSTEMATIC REVIEW AND META- ANALYSIS

Abstract
Introduction
Endoscopic sleeve gastroplasty (ESG) is a minimally invasive procedure that involves the application of full-thickness endoscopic sutures to form plications in the stomach and reduce gastric volume. Data on the learning curve for ESG is not well reported. We conducted a systematic review and meta-analysis to evaluate the learning curves for ESG based on plateau cases required for attaining efficiency.
Methods
A systematic review of the literature from MEDLINE, EMBASE and Scopus was conducted from inception to November 2024, for studies reporting on the learning curve for ESG. Outcomes of interest were the mean length of procedure, plateau of procedure time and corresponding procedure case number. Standard meta-analysis methods were followed using the random-effects model. Heterogeneity was assessed using the I2% statistics.
Results
Five studies were included that reported on 563 consecutive ESG procedures. ESG was performed by single operators in all studies; 2 studies employed endoscopists and 3 involved bariatric surgeons. 3 studies were retrospective in nature while 2 were prospective case series; all studies were done at single centers. Of the 563 patients, 395 (70.2%) were female. Mean age ranged from 40.2 to 47.7 years and mean BMI ranged from 33.6 to 41.8 kg/m2 Pooled analysis showed the number of ESG cases at which procedure time plateau was attained was 23.80 (95% CI 16.97 – 31.35, I2: 52.0%). Pooled mean length of procedure was 91.56 minutes (95% CI 50.96 – 132.16, I2: 99%). These results are summarized in Figure 1.
Conclusion
In this meta-analysis evaluating the learning curve of endoscopic sleeve gastroplasty for first time operators of this procedure, we identified a threshold of 24 cases at which procedure time plateau was obtained. Further studies into the relationship between the learning curve and safety outcomes are warranted.
