Abstract

Objective To determine Lef-1 expression in benign endometria as a function of age, body mass index (BMI) and histology. Design Experimental study. Materials and Methods IRB consent was obtained from patients undergoing hysterectomy for benign diagnoses. Two independent board-certified pathologists examined and confirmed endometrial histology. Total RNA was isolated and Lef-1 gene expression analysis was performed by quantitative rtPCR. Statistical analysis was performed using SAS 9.2. Results 41 benign uteri were sampled. Lef-1 expression was significantly higher in proliferative endometria when compared to secretory (fold-change (FC) 12.63 P<0.04) and inactive (FC 12.51 P<0.01) samples. There was no difference in Lef-1 expression between inactive and secretory endometria. Inactive endometria of overweight, obese, and morbidly obese women had progressively elevated levels of Lef-1 when compared to women with normal BMI (FC of 2.62, 7.30 and 13.88 respectively, P<0.001). Lef-1 was also significantly more expressed in proliferative endometria of normal weight, overweight, obese, and morbidly obese women when compared to women with inactive endometria and normal BMI (FC 9.07, 13.22, 18.05, 27.32 P<0.001). There were no age-related Lef-1 expressional changes. Conclusion Lef-1 overexpression has been detected in several cancers such as pancreatic, colon, leukemia and melanoma. Recently, Lef-1 has been shown to play a role in endometrial gland and carcinoma formation. As hypothesized, we found increased Lef-1 expression in proliferating endometria. This expression increased with increasing BMI. A similar BMI-associated increase in Lef-1 expression occurred in post-menopausal women with inactive endometria. Obesity is a known risk factor for proliferative uterine pathology including fibroids, polyps, hyperplasia and cancer. Obesity related Lef-1 overexpression may be a causative agent of these changes and may signify future disease. A pathogenic role of Lef-1 in these disorders has yet to be elucidated.
