Na+/H+‐EXCHANGER‐1 INHIBITION REVERSES PERIPHERAL DIABETIC NEUROPATHY

Abstract
Evidence for the important role of Na+,H+‐exchanger‐1 (NHE‐1) in diabetic complications is emerging. The aim of this study was to evaluate if NHE‐1 inhibition reverses experimental peripheral diabetic neuropathy (PDN). Control and streptozotocin (STZ)‐diabetic rats were maintained with or without treatment with the specific NHE‐1 inhibitor cariporide for 4 weeks after first 12 weeks without treatment. Rats with 12‐week duration of STZ‐diabetes developed motor and sensory nerve conduction slowing, neurovascular dysfunction manifest by deficit in endoneurial nutritive blood flow and impaired vascular reactivity of epineurial arterioles, thermal hypoalgesia, tactile allodynia, and intraepidermal nerve fiber loss. All these changes were completely or partially reversed by cariporide treatment. NHE‐1 inhibition was also associated with reduced accumulation of advanced glycation end‐products and nitrated proteins in diabetic peripheral nerve. In conclusion, our findings support an important role for NHE‐1 in PDN, and provide rationale for development of NHE‐1 inhibitors and NHE‐1 inhibitor‐containing combination therapies for treatment of this devastating diabetic complication.
