3. Adapt a monitoring plan to optimize safety and effectiveness of methadone
Methadone is an essential and effective medication to treat pain for hospice and palliative care patients. The barriers to using methadone include complexities related to pharmacology, pharmacokinetics, drug interactions, dose conversions from other opioids, and the potential for development of QT prolongation and cardiac arrhythmias. Appropriate patient selection and close monitoring are crucial to ensure safe and effective management of methadone therapy in hospice and palliative care. We aim to guide hospice and palliative care clinicians with interprofessional outpatient-based clinical decision support tools for safe and appropriate use of methadone when initiating therapy and for follow-up visits.
This session will be presented by interprofessional outpatient-based palliative care clinicians. We will review the role and safety of methadone in managing pain for hospice and palliative care patients. The indications and contraindications for initiating methadone and methods for monitoring methadone use will be reviewed by a palliative care pharmacist. We will use simulated case examples to demonstrate which patients may be appropriate or inappropriate for methadone. Lastly, as a group, we will discuss challenges that may result from caring for patients using methadone and how to best collaborate with referring providers.
