Overcoming Hurdles to Video Recording in the Operating Room for Surgical Education

Abstract
INTRODUCTION: Video recording in the operating room (OR) is a promising modality for studying optimal methods of intraoperative surgical education. Unfortunately, many institutions lack formal policies to streamline the approval and regulation process. We describe our implementation of video recording in the OR for educational purposes.
METHODS: We consulted hospital policy makers, lawyers, HIPAA compliance officers, institutional IT, and Health Sciences IRB regarding their concerns around video recording in the OR. Surgery faculty, residents, and researchers were surveyed to determine their interest.
RESULTS: Our faculty and resident survey showed that 79% of the 91 respondents indicated interest in using video recording in the OR. Concerns of consent and confidentiality raised by stakeholders were addressed. Patient surgical consent forms now include consent for video recording for education research. Attending and resident research participants are consented via a standard IRB process. Per IRB request, a NIH certificate of confidentiality was obtained. OR staff not participating in the research are given the opportunity to opt out of working in ORs when video research is being conducted. Additionally, signs are placed on the OR doors to alert those entering that video recording is in progress. Videos are kept confidential within a firewall-protected platform where they can be accessed remotely.
CONCLUSION: Video in the OR for educational research can be successfully implemented in a major academic medical center. Through sharing solutions to implementation, this type of research can be broadly used for future educational work.
