P291 Impact of endoscopy immersive training on service provision and trainees experience within the midlands endoscopy training academy

Abstract
IntroductionShape of training and service restoration following the COVID 19 pandemic have negatively influenced endoscopy training. The BSG TS recently highlighted concerns around endoscopy training. Enhancing endoscopy training and provision of immersive training are objectives of Endoscopy Training Academies. Establishing and evaluation of the quality and impact of immersive training are key in understanding its role as a training offer.We aimed to review the first year of immersion training experience at our endoscopy academy from a service provision as well as trainees experience point of view.MethodsThe service impact of colonoscopy immersive training (2 consecutive days/week for 4 weeks) for 12 trainees over a one-year period was assessed at Cannock Chase Hospital. As a comparison, data for 2 days/week of standard endoscopy (SE) lists from the same week was collected. Then trainee data was assessed for impact of immersive training on trainee colonoscopy metrics during the immersive period with a wider group of trainees. Data was collected via a combination of trainee completed survey and interrogation of the NED.ResultsIn total, 96 days of immersion training (IT) were compared against 97 days of standard endoscopy (SE) lists. During the study period, 192 lists were scheduled with the room utilised for 185 (96%) due to various reasons. In comparison, 212 lists were scheduled for SE and 181 utilised (85%).Abstract P291 Table 1 Lists utilised Lists not utilised Immersion training (IT) 185 7 Standard Endoscopy (SE) 181 31 A chi-square test of independence was performed to examine room utilisation for IT vs. SE. This showed that IT lists had better room utilisation (X2 (1, N = 404) = 14.2464, p = 0.00016).IT was mostly colonoscopy lists (97%) while SE lists were mixed in modality; colonoscopy (48%), mixed (40%) and OGD only (12%) with list utilisation of 87%.12 trainees attended immersion lists during the study period and 8 of them responded to feedback survey. Average of 49 procedures were completed by each trainee during IT. Average cecal intubation rate for trainees was 62% pre-IT compared with 87% during IT. Average terminal ileum intubation rate during IT was 50.02% and polyp detection rate was 49%. Average patient comfort score was 2.Out of those trainees who responded to survey, 62% felt that 4 weeks of immersion training was sufficient. All except one felt that they would repeat immersion training if given the opportunity. Unfortunately, only 33% percent trainee were able to achieve a provisional sign off in 12 months following IT.ConclusionsImmersion training is an effective way to upskill trainees in a short period of time. Provision of IT allowed better room utilisation to a service. Trainees experience is positive with significant improvement in their KPIs. However, IT forms a part of the training journey and ongoing dedicated training lists should be provided in their parent trusts around IT blocks.
