PET-determined Myocardial Blood Flow and Left Ventricular Hypertrophy in Obesity

Abstract
Introduction: The effect of left ventricular hypertrophy (LVH) on myocardial blood flow (MBF) regulation in obesity remains to be further elucidated. Methods: 13N-ammonia positron emission tomography/computed tomography evaluated global myocardial blood flow (MBF) at rest, during pharmacologically induced hyperemia, and corresponding myocardial flow reserve (MFR)(MFR=MBFstress / MBFrest). Patients were grouped according to body mass index (BMI) into normal weight (NW) (BMI <28 kg/m2, n=38), obesity (OB) (BMI 30.0-39.9 kg/m2, n=62), and morbid obesity (MOB) (BMI ≥40 kg/m2, n=78). Echocardiography determined left ventricular function and wall thickness in obese individuals. Only individuals with normal systolic function were included. Septal wall thickness ≥12mm defined left ventricular hypertrophy (LVH). Results: Resting MBF was comparable among groups (NW:0.89±0.19, OB:0.93±0.23, and MOB:0.91±0.2 mL/g/min; p=0.637 by ANOVA). MFR decreased significantly from NW to OB (2.61±0.79 vs. 2.33±0.62 mL/g/min; p<0.05), while it increased again in MOB comparable to NW (2.47±0.52 vs. 2.61±0.79 mL/g/min; p=0.164) (Table 1). When subgrouping according to the presence and absence of LVH in obesity, MFR did not differ significantly between those with LVH than in those without (OB: 2.22±0.50 vs 2.43±0.71 mL/g/min, p=0.178; and MOB:2.52±0.44 vs 2.41±0.59 mL/g/min; p=0.332). MFR did not correlate with septal wall thickness in OB and MOB (r=0.12, SEE:0.676, p=0.178) *<=0.05 vs. NW, BMI: body-mass index, MBF: myocardial blood flow, MFR: myocardial flow reserve, HR:heart rate, SBP:systolic blood pressure, RPP: rate-pressure product Conclusions: The observed U - turn of the MFR obesity to morbide obesity (functional obesity paradox) signifies obesity and morbid obesity to affect coronary circulatory function differently, not affected by LVH, warranting further metabolic and clinical investigations.
