Abstract
Objective: Optilume drug-coated balloon is a relatively new treatment modality for the treatment of urethral stricture disease. The initial ROBUST trials have included routine predilation which allows cystoscopic evaluation proximal to the stricture. Similar to cystoscopy for microscopic hematuria, this helps to rule out concurrent conditions which could impact patient management. However, predilation with a second balloon can increase both cost and procedure length. We sought to categorize the occurrence of unanticipated findings following urethral predilation for Optilume drug-coated balloon.
Methods: A review of all cystoscopy and Optilume drug-coated balloon dilations performed at a tertiary referral center from January 2020 to September 2024 was performed. There was a total of three high volume reconstructive urologists who routinely performed predilation using a 24Fr non-coated balloon, followed by the Optilume drug-coated balloon. Following predilation complete cystourethroscopy was performed. Location of stricture, preoperative evaluation, whether there were unexpected findings during initial predilation. Descriptive statistics were then performed.
Results: A total of 128 dilations were performed during the time frame. Median patient age was 65 (range 18-97, IQR 19.25). The majority of the strictures were within the bulbar urethra (95/128, 74.2%) and the majority of patients underwent preoperative evaluation with retrograde urethrogram (81/128, 63.3%). 13 of the 128 procedures had unexpected findings (10.1%). Seven patients had a second stricture, 3 patients had bladder stones, and 2 patients had tumors.
Conclusion: Ten percent of cases of cystoscopy and Optilume drug-coated balloon dilation had secondary findings changing overall management plan for patients. This speaks to the importance of continuing to incorporate predilation with the use of the Optilume drug-coated balloon.
