Abstract
OBJECTIVE: Primary fallopian tube cancer is an uncommon malignancy. The aim of this study was to identifying prognostic factors and markers that influence clinical outcome of primary fallopian tube cancer at a large tertiary care center.
METHODS: A retrospective review of the tumor registry was conducted between 1977 and 2007. Primary endpoints were overall survival and disease recurrence.
RESULTS: There were 53 patients with primary fallopian tube cancer and median age of 65 years. Patients most frequently presented with an abdominal or pelvic mass in 21% and abdominal bloating in 22%. The most common histology was serous papillary adenocarcinoma in 39.6%.
Poorly differentiated tumors were diagnosed in 60%. The most common stage was stage III (37.3%). The median follow– up was 40 months. The 5-year disease-free survival rate was 45%, and the overall survival rate was 81%. By the log rank test, the probability of disease-free survival was related to International Federation of Gynecology and Obstetrics stage (early 92% compared with advanced 22%, P.01) and serum CA 125 (75% less than 35; compared with 28% more than 35, P.04). The most common location of recurrence was pelvic and abdominal (60%). The probability of overall survival was related to International Federation of Gynecology and Obstetrics stage (early 92%
compared with advanced 34%, P.01), and residual tumor (64% optimal compared with 16% suboptimal, P.001). Multivariate analysis showed that advanced stage (P.03) and high grade (P.05) were independent prognostic factors for disease-free survival. Additionally, advanced stage (P.07) and optimal tumor debulking (P.016) were independent prognostic variables for overall survival.
CONCLUSION: Fallopian tube malignancies are rare. Additional studies should continue to be performed to help elucidate the distinct causative, diagnostic, and treatment guidelines for primary fallopian tube cancer.
