Prophylactic Use of Antibiotic Beads Is Not Associated With Decreased Groin Wound Complications After Vascular Procedures

Abstract
Objectives
Antibiotic beads are commonly utilized during hardware placement in orthopedic and neurosurgery procedures. In vascular surgery, their use has typically been limited to infected wounds. This study aimed to evaluate whether the prophylactic use of antibiotic beads during femoral cutdown procedures decreases the rate of groin wound complications.
Methods
We retrospectively reviewed patients who underwent vascular procedures requiring a groin femoral cutdown at our center between 2024 and 2025. Patients with preoperative wound infections or multiple ipsilateral incisions (eg, bypass) were excluded. Patient demographics and outcomes were compared using the χ2 test for categorical variables and independent t tests for continuous variables. A multivariable logistic regression was performed to assess the association between prophylactic antibiotic beads use and postoperative wound complications, including cellulitis, deep infection/abscess, and lymph leak.
Results
A total of 107 patients met the inclusion criteria: 57 received prophylactic antibiotic beads, while 50 did not. Baseline characteristics were similar between groups (Table). The overall wound complication rate was 14%, with no significant difference between the beads and no-beads groups (15.7% vs 12.0%; P = .57). Multivariable analysis, adjusted for age, smoking status, body mass index, diabetes, and operative time, revealed no significant association between bead use and wound complications (P = .89).
Conclusions
In this single-center retrospective study, the prophylactic use of antibiotic beads after femoral cutdown was not associated with a lower incidence of postoperative wound complications in vascular surgery patients.
