Abstract
OBJECTIVE
To assess the impact of ejaculatory abstinence (EA) length on fertilization rate in IVF cycles.
MATERIALS AND METHODS
Patients undergoing their first IVF cycle using standard insemination or ICSI from January 2021 to 2022 were included. Men reported the number of EA days prior to semen collection. Those with an EA period of 2 to 7 days were included. Patients were excluded from analysis if donor or frozen sperm were utilized. Our primary outcome was the fertilization rate, calculated by #2PN/# of oocytes inseminated. Our secondary outcomes included the semen analysis (SA) parameters, clinical pregnancy, miscarriage and live birth rate.
RESULTS
An increasing EA length was associated with improved SA parameters including volume, sperm concentration, total sperm count, total motile sperm count (TMSC). Despite an increasing TMSC with longer periods of EA, there was no effect of increasing EA on either ICSI or standard insemination fertilization rates. There was no significant difference in clinical pregnancy (OR 1.00, 0.87 – 1.15), miscarriage (OR 1.16, 0.93 – 1.44), or live birth rates (OR 0.95, 0.83 – 1.09), with increasing periods of EA.
CONCLUSIONS
While increasing days of EA is associated with improved SA parameters, there was no effect of increasing EA days on either ICSI or standard insemination fertilization rates, clinical pregnancy, miscarriage, or live birth rates.
IMPACT STATEMENT
This study may provide assurance when counseling patients that are questioning what the best EA period within the standard 2 – 7 day period is for fertilization outcomes.
