Abstract
Background
A number of findings indicating submucosa(SM)-massive invasion (>1000μm) in early-stage colorectal cancer(CRC) are known, but there are very few reports focusing attention on blood vessels in the tumor periphery. We defined abnormal blood vessels in which there is a possibility of involvement in invasion depth as “blood vessels in the tumor periphery that run through 2 or more glands, with type I pit as the background”, and we thus performed a retrospective study.
Objectives and Methods
The objective was to study the relationship between abnormal blood vessels in the tumor periphery and invasion depth of the lesion. Among high grade dysplasia(HGD)/SM slight(SM-s)/SM massive(SM-m)/the proper muscle(MP) diagnosed at our hospital between January 2012 and August 2014, we verified the presence of the abovementioned abnormal blood vessels in 376 lesions (HGD:210, SM-s:35, SM-m:94, MP:37) having a diameter within 3 cm, in which observation of the lesion could adequately be performed.
Results
Abnormal blood vessels were observed in 3/210 cases (1.4%) of HGD, 7/35 cases (20.0%) of SM-s, 47/94 cases (50.0%) of SM-m, and 34/37 cases (91.9%) of MP. All lesions with abnormal vessels were depressed, except 2 cases, which are SM-m and MP with 0-Is.
Conclusion
Abnormal blood vessels in the tumor periphery indicate that there is a correlation with invasion depth in depressed lesions. In invasion depth diagnosis, even in cases where observation of the pit pattern of the lesion is difficult, if abnormal blood vessels are seen in the tumor periphery, then it is important to suspect invasion at the SM-massive or deeper.
