Abstract

Background Myocardial infarction (MI) is a major concern. We analyzed the association of AHAʼs Life Essential 8 (LE8) with MI in the US. Methods We used NHANES data (2013-2020) for individuals ≥20 years and calculated LE8 scores using AHAʼs updated definitions. LE8 scores were categorized as low (<50), moderate (50-79) and ideal (≥80) CVH. Univariate and multivariate sample-weighted logistic regressions were used to analyze the association between CVH categories and MI. The multivariate model adjusted for age, gender, race, marital status, education, alcohol consumption and family history of CVD. Subgroup analysis was conducted for males (M) and females (F). Results Among 35706 NHANES participants, 15867 had complete LE8 data. Of these, 1866 had low CVH (M: 46.25%, F: 53.75%), 10933 moderate (M: 51.21%, F: 48.79%) and 2888 ideal CVH (M: 40.69%, F: 59.31%). The unadjusted weighted OR of a history of MI was 0.41 [95% CI: 0.31-0.56, p < 0.001] for moderate and 0.08 [95% CI: 0.05-0.13, p < 0.001] for ideal CVH compared to low CVH. In multivariate analysis the adjusted weighted OR for moderate CVH was 0.54 [95% CI: 0.35-0.85, p < 0.001] and 0.21[95% CI: 0.12-0.39 p < 0.001] for ideal CVH. In males the adjusted weighted OR was 0.56 [95% CI: 0.32-0.99, p < 0.05] for moderate and 0.24 [95% CI: 0.12-0.52, p < 0.001] for ideal CVH. In females it was 0.51 [95% CI: 0.28-0.94, p < 0.001] for moderate and 0.15 [95% CI: 0.05-0.49, p < 0.001] for ideal CVH. Conclusion LE8 inversely correlates with MI risk. Optimizing LE8 may reduce MI burden.
