Abstract

OBJECTIVE The miscarriage rate following frozen embryo transfer (FET) with in vitro fertilization (IVF) cycles has not been extensively studied. Therefore, we decided to analyze the temporal trend of miscarriage rates following IVF FET on a year-by-year basis and estimate the risk ratio for miscarriage for multiple demographic and treatment variables in singleton pregnancies which resulted from FET in the United States. MATERIALS AND METHODS Miscarriage data for women (age 18-49 years) with singleton pregnancies resulting from FET using autologous oocytes were obtained from the Society for Assisted Reproductive Technology Clinic Outcome Reporting System. We calculated the miscarriage rate in the whole sample and performed a modified Poisson regression model to estimate risk ratios for miscarriage based on each additional year from 2014 to 2022, as well as those for multiple demographic and treatment variables. RESULTS A total of 294,586 singleton pregnancies (mean [SD] age of 33.75 years [4.11] and BMI of 26.15 [5.88]) resulting from FETs performed January 2014 through December 2022 were reported. 59.7% patients underwent PGT analysis. The prevalence of clinical miscarriages in the whole sample was 10.2%. There was no change in the risk of miscarriage with each additional reporting year. In our model, the independent predictors that showed statistically significant effect on clinical miscarriage rate included maternal age, BMI, multiple embryo transfer, and the use of preimplantation genetic testing (PGT). CONCLUSIONS The nationwide miscarriage rate from 2014 to 2022 remained stable in women with singleton pregnancies after a frozen embryo transfer using autologous oocytes. IMPACT STATEMENT Findings from this large, national retrospective cohort study demonstrates that the trend of miscarriages remained stable over eight years. Furthermore, the use of preimplantation genetic testing was associated with statistically significant reduction in miscarriage rates.
