Unmedicated major depressive disorder is associated with neuroinflammation

Abstract
Objectives: Neuroinflammation is associated with major depressive disorder (MDD) and can be quantified with PET and radioligands targeting the translocator protein (TSPO). TSPO is a mitochondrial protein which is highly expressed in glial cells of the brain and, therefore, is a biomarker of neuroinflammation. Previous PET and BOLD-MRI findings showed activity changes in the subgenual prefrontal cortex and anterior cingulate cortex of MDD subjects. The aim of this study is to assess whether TSPO binding with [11C]PBR28 in these two brain regions is increased in subjects with MDD and whether this increase is modified by antidepressant therapy and correlated to a clinical depression rating scale. Methods: Unmedicated MDD (n = 12; MADRS score = 31.4 ± 3.6; age = 34 ± 9), medicated MDD (n = 16; MADRS = 31.0 ± 4.4; age = 45 ± 10) and healthy controls (n = 20; MADRS less than 1; age 32 ± 10) subjects underwent PET imaging using [11C]PBR28. Total volume of distribution corrected for the plasma free fraction (VT/fP) was calculated with an arterial input function. VT/fP values were correlated to the MADRS scale. Results: In the anterior cingulate cortex, VT/fP was 36% higher in unmedicated MDD subjects compared to HCs (p<0.01) and 25% higher compared to medicated MDD subjects (p=ns). In the subgenual cortex, VT was 38% higher in unmedicated subjects compared to HCs (p<0.01) and 24% higher compared to medicated MDD subjects (p=ns). VT/fP did not correlate with MADRS scores. Conclusion: These preliminary results suggest that inflammation in the anterior cingulate and subgenual cortex is associated with major depression and is likely affected by the medication status of the patient.
