Validation Of The Promis Pain Intensity SF 3A And Brief Pain Inventory Measures During Pregnancy

Abstract
Validated pain measures are needed in pregnancy to promote the accurate measurement of pain and strengthen clinical outcomes. The purpose of this psychometric evaluation is to provide normative data and validation of two commonly used pain measures– PROMIS Pain Intensity SF 3a and Brief Pain Inventory (BPI) pain intensity scale during pregnancy. One hundred and seventy-one pregnant women were prospectively surveyed before 16 weeks gestational age and 123 pregnant women were surveyed during their third trimester of pregnancy, 98 of these women were surveyed longitudinally. The women had a mean of 31 years of age, were predominately White (94-96.5%), and married (83%) with private insurance (82%). Both the PROMIS Pain Intensity and BPI correlated with other measures of pain, but did not with non-pain measures, indicating convergent and discriminant validity. Test criterion validity results indicate both measures were effective in measuring the worst, average, and current pain intensity of back and other types of pain before 16 weeks and after 28 weeks gestational age (p<.001). Head pain was not significant during the third trimester of pregnancy in either measure possibly due to the normal physiologic changes of pregnancy. Our results suggest that both the PROMIS Pain Intensity SF 3a and BPI are valid measures of pain intensity during early and late pregnancy. Research is needed to further validate these measures in diverse samples of women, across additional time points of pregnancy, and in response to the treatment of pain to enhance clinical outcomes.
