Abstract
Objective: Commonly in the buried penis repair surgical procedures, the penile skin is unhealthy and requires excision. Coverage after excision can be provided with local flaps or skin grafts. Flaps can offer many advantages over skin grafting, including reduced morbidity and simplified postop care. Here, we present our novel technique for Bilateral Penoscrotal Fasciocutaneous Transposition Flap, which we have found to be reproducible in this patient population.
Methods: The video presents a case of a 63 years old gentleman who has a history of acquired buried penis secondary to a large escutcheon. He underwent buried penis repair with escutcheonectomy (not shown here) along with penile skin excision and substitution with a local fasciocutaneous flap that is described in this video
Results: All the surgical steps are well documented and described, demonstrating the procedure in the smallest details. Critical steps include: Penile skin excision and degloving, median raphe incision, transposition and fixation of flap dorsally, wrapping of ventral side with lateral incision to reestablish penoscrotal angle followed by multilayer wound closure.
Conclusion: Our video attempts to present a simple and easily reproducible technique for provide flap coverage. The bilateral fasciocutaneous scrotal transposition flap technique is a versatile, reproducible method for penile coverage during buried penis repair. We have found the technique to be particularly useful in morbidly obese men with an intact pannus that is not addressed or removed at the time of repair, as managing STSG’s can often be challenging. We believe the technique described here provides a more favorable alternative in this patient population.
