Abstract
Pupillary and eyelid abnormalities are often encountered in clinical practice. In this chapter, we begin by reviewing the anatomy and neural control of the iris muscles. We then describe normal pupil phenomena. We next review abnormalities of the pupil secondary to lesions of the afferent segment of the pupillary light reflex (i.e., the relative afferent pupillary defect). Then, we review abnormalities of the pupil secondary to lesions of the efferent segment, including those affecting the sympathetic innervation, causing Horner syndrome with pupillary miosis, and those affecting the parasympathetic innervation, causing pupillary mydriasis. In the second section of the chapter, we review the anatomy and neural control of the eyelid muscles. We next review common abnormalities of the eyelid, including eyelid ptosis, eyelid retraction, and dynamic disorders of the eyelids such as blepharospasm, apraxia of lid opening, and hemifacial spasm.
