Abstract

Obliterative surgery or colpocleisis is performed in approximately 5% of women in the United States undergoing surgery for pelvic organ prolapse. The surgery is most often undertaken in the elderly or comorbid individual with advanced-stage prolapse who is willing to forego penetrative vaginal intercourse, as it is an effective procedure with quick recovery and low morbidity. However, given these benefits it can also be offered to any patient who may not wish to preserve a functional vaginal length. Obliterative surgery can be performed with or without hysterectomy in patients with uterovaginal prolapse and is an option for patients with posthysterectomy prolapse. Obliterative surgical techniques include the Le Fort colpocleisis (partial colpectomy) or complete colpectomy and colpocleisis. Often, an extensive perineal repair with or without levator plication is performed with either approach to narrow the genital hiatus. This chapter discusses the indications, techniques, and outcomes for these procedures.
