Abstract
This chapter provides a summary of the landmark study known as a comparison of four methods of weaning patients from mechanical ventilation. What is the best method for weaning mechanical ventilation in patients with an initial unsuccessful spontaneous breathing trial? Starting with that question, the chapter describes the basics of the study, including funding, study location, who was studied, how many patients, study design, study intervention, followup, endpoints, results, and criticism and limitations. The chapter briefly reviews other relevant studies and information, discusses implications, and concludes with a relevant clinical case. Among carefully selected patients who were difficult to wean from the ventilator, a oncedaily trial of spontaneous breathing led to extubation about three times more quickly than intermittent mandatory ventilation and about twice as quickly as pressuresupport ventilation.
