Abstract
Pulmonary cystic echinococcosis (CE) is uncommon in non-endemic countries and may be difficult to diagnose and manage. We describe a case of a rupture CE mimicking complicated empyema in a pregnant patient.
Pulmonary cystic echinococcosis is common in endemic areas of the world, but it is rarely seen in other regions. Given the increase in worldwide travel and migration, all thoracic surgeons must be aware of hydatic disease and the principles of its management. Chest radiography demonstrated right lower lobe opacification with a large ovoid collection with an air-fluid level. Low-dose unenhanced chest computed tomography demonstrated an 8-cm air-fluid collection along the posterior medial aspect of the right lower lobe with consolidation in much of adjacent lung parenchyma. Thoracoscopic drainage of empyema and decortication were performed with adequate re-expansion of the lung. Ruptured and infected cysts are managed according to the same principles of complete cyst removal and maximal eradication of the parasite. Drainage of empyemas, decortication of the lung, and pericystectomy are performed as needed. Closure of visible bronchial openings is performed but without capitonnage.
