Abstract
Cardiac sarcoidosis occurs throughout the world, affecting all genders, ethnicities, and ages. Diagnosis can be difficult, particularly in those with sarcoidosis confined to the heart. Although a rare disease, the exact incidence and prevalence of cardiac involvement are likely underestimated due to its subclinical presence or manifestations that overlap more common cardiac ailments. Additionally, the observed prevalence appears to be increasing over time, likely a result of advancing imaging technology and increased awareness of cardiac manifestations. The presence of granulomatous involvement in the heart has potentially lethal consequences. It important to better understand the epidemiology of this disease because early detection and treatment may decrease morbidity and mortality in patients with sarcoidosis.
Epidemiologic studies regarding the incidence, prevalence, and mortality rates of cardiac sarcoidosis are difficult to conduct for a variety of reasons. Accuracy of epidemiology surveys are limited by the rarity of the disease, non-standardized case definition or lack of global gold standard, heterogeneity of screening procedures, and differences in case presentation. Further, regional variability in phenotypic presentation and baseline prevalence can make estimates hard to compare across populations. Reporting of data from differing sources such as registries and insurance databases are also problematic to compare, given varying data collection methods. Despite these limitations, significant progress has been made in defining the extent of disease burden and determining predictive factors for outcomes.
