Abstract
Breast cancer care should be approached in a multidisciplinary fashion with surgeons at the center, coordinating care with radiology, medical oncology, radiation oncology, genetics, and psycho-oncology. The diagnosis should be made by needle biopsy in most cases, although sometimes an excisional biopsy may be warranted. Survival is no different for breast-conserving therapy (BCT) and mastectomy and either is acceptable for most cases of invasive breast cancer and ductal carcinoma in situ. The role of axillary surgery as a staging and therapeutic intervention is under constant reexamination, balancing the need for information, the efficacy in treating known axillary disease, and the potential negative impact on quality of life. Sentinel lymph node biopsy is now standard with complete axillary lymph node dissection being saved for specific circumstances. Genomic information is now routinely used to tailor individual treatment plans. This chapter summarizes key treatment patterns and the surgical management of invasive breast cancer, ductal carcinoma in situ (DCIS), and related histopathologies. It also discusses the management of benign breast problems such as abscesses and fibroadenomas.
