Abstract
Deep brain stimulation has become the standard therapeutic option for patients with advanced Parkinson’s disease. It involves implantation of a stimulation electrode in the deep structures of the brain, often the subthalamic nucleus or the globus pallidus interna. The stimulation electrode provides continuous electrical stimulation that ameliorates the symptoms of Parkinson’s disease, especially tremor, rigidity, bradykinesia, and most importantly, motor fluctuations. Axial and nonmotor symptoms tend to be resistant to this technique when applied to classical targets, but new targets are being explored to overcome these shortcomings. Proper case selection is imperative for favorable outcome after surgery. In this chapter we will discuss the most important aspects of deep brain stimulation surgery, perioperative management, and long-term outcomes.
