Abstract
Neuroimaging plays a pivotal role in the evaluation of patients with epilepsy. By identifying and characterizing a brain lesion at the epileptic focus, imaging assists in the diagnosis and management of patients. This role is best exemplified by the presurgical assessment in epilepsy patients. Lesions known to be cause of seizures tend to have characteristic imaging presentations. These include hippocampal sclerosis, malformations of cortical development, neoplasms, and vascular lesions. More recently described entities such as amygdala enlargement and immune-mediated encephalitis are also presented. DWI adds value to epilepsy evaluation particularly by detecting different patterns of postictal cytotoxic edema. Furthermore, DTI is routinely used as part of presurgical brain mapping.
