Health, place, and justice: A philosophical appraisal of promoting equity in Covid-19 through disadvantage indices

Abstract
More disadvantaged populations, who comprise larger shares of racial and ethnic minorities, have been hit disproportionately harder by Covid-19 in terms of unemployment, infections, hospitalizations, and deaths. This chapter evaluates some of the ways in which the United States attempted to improve equitable access to healthcare resources in light of those disadvantages and vulnerabilities. This chapter describes a major shift in how equity was sought to be promoted in national and sub-national allocation frameworks, by considering differences among groups not only in the prioritization across different priority populations, but also within each, using place-based measures of disadvantage, such as the Centers for Disease Control and Prevention’s Social Vulnerability Index. We describe their design and conceptualization of layers of disadvantage and compare them with a prominent model of disadvantage articulated in the philosophical literature by Jonathan Wolff and Avner de-Shalit. We close with how, practically, public health was advanced through using these indices, and outline applications in other areas of public health policy.
