Robotic-Assisted Excision of Urachal Adenocarcinoma with Umbiliconeoplasty

Abstract
The urachus is the embryological remnant of the allantois and provides ligamentous attachment from the dome of the bladder to the umbilicus. Both benign and malignant processes affect the urachus. Urachal carcinoma is a rare condition with an overall poor prognosis. Surgical resection is the mainstay of therapy, as chemotherapy and radiation have largely been shown to be ineffective. Initial surgical approaches included radical cystectomy, but partial cystectomy has proven an effective oncologic procedure. Minimally invasive laparoscopic procedures utilizing robotic assistance now accomplish the same surgical goals while minimizing pain, morbidity, and recovery time. Umbilical resection helps ensure negative margins in all cases of malignancy. After cancer control, attention is turned to restoration of the umbilicus, the key feature of the abdominal wall.
