- Mapping the Patient Journey was peer reviewed by program participants who have experienced the process from patient screening to the completion of community-based programming. In Collaboration: Our project stemmed from the IHI/Pfizer Collective Action Collaborative. In partnership with the Institute for Healthcare Improvement and Pfizer, the success of Primary Health Care and The Iowa Community HUB’s Patient Journey Map was created. The project was created and facilitated by: * Primary Health Care: Kay Switzer - Population Health Program Manager, and Teresa Fisher, RN * The Iowa Community HUB:Trina Radske-Suchan, President/CEO and Renee Allard, HUB Administrator. Mapping the Patient Journey was peer reviewed by a panel of patients who completed the full process, from clinic screening through participation in community based programs. Participants provided feedback through a series of phone questions conducted by a HUB representative, which PHC and the HUB reviewed and are using to implement supportive improvements as the care model continues to evolve. The Journey Map was supported and reviewed by Dr. Bery Engebretsen alongside Dr. Jimmy S. Reyes, PhD, DNP, AGNP, RN. Sourcing: The patient journey mapping tool has transitioned significantly over the years. Original sourcing is assigned to the PopHealth Learning Center.
