Muscatine Ponderosity Family Study

Abstract
A family study was conducted in Muscatine, Iowa, to examine the risk of cardiovascular disease in adult relatives of children with different patterns of ponderosity (body weight relative to height), and to estimate the genetic contribution to familial clustering of levels of ponderosity. Four groups of probands were selected from students who participated in three consecutive biennial school surveys: a random group (n = 70); a lean group (n = 72); a gain group (n = 70); and a heavy group (n = 72). The parents, siblings, a related aunt or uncle, and a first cousin of these probands were examined. Death certificates were obtained for deceased adult relatives. The relative risk of dying from cardiovascular disease for the heavy group versus the random group relatives was 1.41 (95% confidence interval 1.01, 1.98). Complex segregation analysis indicated strong support for a single recessive locus with a major effect that accounted for almost 35% of the age-gender-adjusted variability in body mass index. Genotypic probabilities were assigned to family members, and the effect of the recessive major locus on other measures of body habitus and cardiovascular risk factors was examined. This analysis suggested that individuals whose obesity may be caused by a gene at a single genetic locus are at higher cardiovascular risk because of mechanisms associated with their risk factor profile. We are currently searching for genetic factors responsible for the variability in body mass index. This may lead to early identification of individuals and families at high risk for the development of the chronic diseases associated with obesity.
