Background: Cystic Fibrosis (CF) is a common and life-threatening disease in Caucasian populations. Decreased pulmonary function and exercise intolerance are common symptoms, with maintenance of pulmonary function a crucial aspect of care. Aerobic exercise has systemic benefits, such as improved/maintained pulmonary function, though specific dosing parameters for the prescription of aerobic exercise in the pediatric CF population are limited. Thus, the purpose of this case report was to highlight specific dosing parameters of an aerobic exercise intervention used to treat an acute exacerbation of cystic fibrosis in a pediatric acute care setting. Case Description: The patient was an 18 y/o female admitted to an acute pediatric setting with an acute exacerbation following medication noncompliance. Medical care comprised antibiotic therapy and resumed adherence to home medication regimen. Intervention: The physical therapist prescribed an aerobic exercise intervention of treadmill walking with specific dosing parameters including duration of 30 minutes and intensity of 70-80% of the patient’s maximum heart rate. Individualization and adjustment based on subjective reports and objective monitoring were crucial aspects of implementation. This patient was discharged home following improvement of forced expiratory volume to baseline, which was 65% of predicted based on age. Discussion: This case report demonstrates an example of an evidence-based aerobic exercise training program for a pediatric patient with CF in an acute care setting, with further discussion provided on recommended dosing parameters and alternative means of providing an aerobic challenge.
