Chronic Impingement and Muscle Disuse Atrophy – Rehabilitation Considerations after Total Hip Arthroplasty: A Case Report

Abstract
<p>Background: Coxa profunda is a type of femoroacetabular impingement that may lead to early onset<br />hip osteoarthritis and resulting activity limitations. Due to the nature of the condition, surgical treatment<br />involves performing a total hip arthroplasty. Purpose: The purpose of this case report is to highlight<br />how chronic musculoskeletal pain, hip impingement, and muscle disuse atrophy changes the<br />rehabilitation management after bilateral total hip replacements in an individual with past medical<br />history significant for severe coxa profunda. Case Description: This case report describes a 47-yearold<br />male with past medical history significant for coxa profunda who is status post bilateral total hip<br />arthroplasty. Due to chronic impingement of bilateral hip joints, the patient was unable to perform hip<br />flexion greater than 90 degrees. Even after surgical intervention, the patient was still unable to flex the<br />hip to a level that allowed for participation in activities of daily life. The patient’s presentation upon<br />physical therapy evaluation demonstrated consistencies with his history of chronic impingement and its<br />subsequent muscle disuse atrophy. Outcomes: The primary outcome measure used to quantify<br />progress was the lower extremity functional scale, which improved by 17 points over the course of care.<br />The patient was able to increase bio-motor abilities and reach his goals. Discussion: This case report<br />brings to light the debilitating impact chronic weakness can have, even after the deformity was<br />corrected with a routine procedure. Additionally, it will express the importance of gathering a thorough<br />subjective history of condition to act upon. This will not only reduce the disability but improve upon the<br />ability of the patient using a multimodal and holistic approach.</p>
