Background: As the United States experiences a demographic shift with an increasingly aging population, the demand for effective healthcare strategies becomes critical, particularly in addressing the challenges of injury recovery in the geriatric demographic. The purpose of this study is to highlight the conservative rehabilitation management focused on quality-of-life (QoL) improvements for an older individual with chronic rotator cuff injury through physical therapy. Case description: The research centers around a case study involving a 92-year-old male (Patient A) with a six-month-old rotator cuff tear, exploring the intricacies of age-related and QoL factors. The six-week physical therapy intervention aimed to enhance range of motion (ROM) and strength, assessing outcomes through functional tasks, compensation strategy reduction, and improved shoulder mobility. Discussion: While Patient A exhibited positive outcomes in functional tasks and reduced compensation, achieving symmetrical ROM and strength remained below symmetrical values. Factors influencing the results, such as age, the chronic nature of the injury, and conservative versus surgical treatments, were explored. Patient A's personal goals, aligning with the principle of patient collaboration, emphasized the importance of individualized objectives and patient satisfaction over specific measurement increases. The study concludes by proposing Patient A's case as motivational evidence for geriatric populations to consider conservative approaches, integrating personal goals and lifestyle considerations into rehabilitation decisions.
