Background: Description of physical therapy intervention for conservative management of groin pain and osteitis pubis is lacking in current literature even though physical therapy is considered the primary management tool before resorting to surgical intervention. Case Description: A 24-year-old recreational soccer athlete (Patient A) presented to PT with lower abdominal, adductor, and pubic symphysis pain. Interventions included symptom management via rest, ice and progressive tendon loading to correct muscular imbalances and facilitate tissue healing. Outcomes: The patient was treated for a total of 5 visits before initiating return to run progression. Discussion: This case study describes interventions based on progressive tendon loading and the physical stress theory to induce positive adaptions at a muscular level and induce tissue healing.
