Background: In the rehabilitation of neurological conditions, it is important to have literature that describes the progression of the disease, especially in rare cases. Critical Illness Myopathy (CIM) and Guillain-Barre syndrome (GBS) are both neurologic illnesses that have been sparely researched in the rehabilitation realm and the presentation during recovery is not well documented. It is especially rare when these conditions occur in the same patient. Similar physical manifestations in these two diseases can make them hard to discern observationally. Purpose: The purpose of this case is to highlight the recovery process in a patient with CIM and a history of GBS syndrome. In this unique case, the patient refused the lumbar puncture test leading to the inability to definitively differentiate between GBS and CIM. Case Description: A 57-year-old Hispanic female was admitted to inpatient rehabilitation following extubation. The patient presented with the diagnosis of Critical Illness Myopathy (CIM) and a history of Guillain-Barre, hypertension, and a 37-year smoking history. Outcome Measures: The Functional Independence Measure (FIM) levels of assistance were used during the patient’s inpatient rehabilitation stay. The BERG balance assessment, 5 time sit to stand, 6-minute walk test, and single leg stance balance measure were used during outpatient rehabilitation. Manual muscle testing was used to assess strength in both inpatient and outpatient rehabilitation. Discussion: With standard rehabilitation care for a patient with CIM the patient showed improvement in all outcome measures. This case study hopefully demonstrates the limitations present in a patient with CIM subsequent development of GB and what interventions were used in returning the patient to her goals.
